Consider Different Cutoffs When Reporting DaT SPECT Scan in PD vs ET, DLB vs AD
Background: Datscan ® with Ioflupane labeled with iodine-123 ( 123 I) is increasingly being used to distinguish 123 I-ioflupane accumulates in the striatum. In patients with Parkinson disease (PD) and related diseases, and in patients with dementia with Lewy bodies (DLB), there is a loss of nerve cells in the striatum. Quantitative software has been developed to assess uptake in the striatum, caudate, and putamen. Objective: To identify the most accurate z-score cutoffs for striatal binding ratio (SBR) of striatal and substriatal regions to discriminate PD versus essential tremor (ET) and DLB versus Alzheimer disease (AD). Participants/Methods: 225 patients undergoing Datscan were enrolled (75 PD, 80 ET, 50 DLB, and 20 AD). Semi-quantification was computed by means of Datquant ® software, which measures striatal SBR and z-scores with respect to 118 healthy volunteers. Receiver-operating characteristic analysis was used to identify most accurate cutoffs for z-score for striatum and su
