TAVR, SAVR Yield Similar Echocardiography, Clinical Outcomes at 5 Years
Background: Transcatheter aortic valve replacement (TAVR) is an acceptable alternative to surgical aortic valve replacement (SAVR) for patients with severe, symptomatic aortic valve stenosis across the spectrum of operative risk. The PARTNER 3 (Safety and Effectiveness of the SAPIEN 3 Transcatheter Heart Valve in Low Risk Patients with Aortic Stenosis) trial compared TAVR to SAVR in low surgical risk patients and found no difference in the composite end point of death, stroke, or rehospitalization related to the procedure, stroke, or heart failure at 5 years. The clinical impact of echocardiogram changes at 5 years is uncertain. Objective: To compare the echocardiographic changes in TAVR versus SAVR, and to determine the clinical outcomes at 30 days and 5 years. Participants: 1000 patients with severe, symptomatic valvular aortic stenosis (AS) who were at low surgical risk were enrolled between 2016 and 2017. Methods: 71 centers randomized low-surgical-risk patients to TAVR with the
